The primary purpose of the Clinical Nurse Specialist Grade 2 (CNS2) in the Diabetes Centre is to provide person centred, evidence based diabetes education and management to patients and carers in an outpatients setting. The CNS2 will also coordinate and assist with the Diabetes Centre Outpatients Clinics. The CNS2 is a resource and clinical expert in the area of diabetes education and management and will work with the Clinical Nurse Consultant Diabetes to provide additional support to the effective coordination of inpatient and staff education. Where you''ll be working Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. POWH provides an exceptional staff experience and enhanced patient and family outcomes as a result of the provision of person-centred care. Selection Criteria Current registration with the Nursing & Midwifery Board of Australia (AHPRA) and current NSW Drivers License (if required by service). Holds postgraduate qualifications in diabetes nursing and at least 3 years recent experience working in diabetes education management; and credentialed with the Australian Diabetes Education Association or such other qualifications or equivalent experience deemed appropriate. Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans. Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations. Demonstrated skills in the development & facilitation of patient and carer education and staff as required. Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others. Detailed knowledge and understanding of harm minimisation and evidence of leadership in safety and quality innovation. Demonstrated competence in the use of healthrelated information technology (e.g. insulin pump, CGMS and flash glucose monitoring).
