Job Description

Access Support Coordination (ASC) are specialists in providing their clients with the right access to supports to accelerate their independence and achieve their goals.About this opportunity:The Support Coordinator is responsible for connecting participants with a NDIS Plan to formal and informal services and supports in their community and assisting participants in understanding and reviewing their NDIS plan.Key responsibilities of this role:Build the capacity of the participants to achieve greater independence to self-direct services and support in the long termEnsure mainstream services meet their obligations and organisational responsibilityProvide the participants and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencyOversee the implementation of service agreements for participants including funding capacityAssist participants to understand service options and to make decisions in relation to preferred optionsLink participants to service providers, and support participants to obtain quotes and establish service agreements with selected providersEnsure evidence is collated to improve client outcomesDevelop relationships with key community stakeholders & liaise with other statutory bodies and government departmentsInvestigate suitable options for service users in the way of recreational, career orientation or accommodation optionsTo succeed in this role you will have:Experience in a Support Coordination role or similar for a minimum of 2 years (specifically with mental health is highly desirable)Certificate IV in Disability or other relevant qualification (highly desirable)Strong knowledge of local disability community and providersAn understanding of billable hours in relation to KPI targetsBasic accounting and budgeting skillsExcellent written and verbal communication skillsWorking with Children Blue Card and NDIS Worker Screening/Yellow CardDrivers LicenceWhy work with us:Flexible work options flex day modelAnnual FREE Flu VaccinationAcademic Study Day ProgramSonder complete care platform 24/7 supportOngoing learning and development including career growth opportunitiesCorporate Health Care PartnersTemporary Office Swap locations across QLDRecognition Employee of the Month, 5 & 10 year Service AwardsSanctuary Model of Care framework trainingCRUX Management Services (CRUX) provide people, compliance, and corporate services to a select number of companies.
