We are building our team and seeking passionate people who are keen to be a part of a thriving and friendly atmosphere.We areseeking an experienced, enthusiastic and highly motivated Care Coordinator to coordinate, manage and administer the delivery of care services to our clients. Our clients are a mix of Private, Home Care Package & NDIS.This role is responsible for:Conducting client care assessments and care planningManaging client compliance and a case load of clientsIntake processing, clinical care planning and reportingCoordinate the provision of quality person centred care according to the client''s needs, service/care plans and available funding in accordance with budgetGeneral administration with respect to tasks performedSkilled nursing in the community as requiredPreferred Skills:Previous experience within the aged care/NDIS/mental healthCare coordinationrole is desirable.Knowledge of My Aged Care, Home Care Package and NDISMust be an Enrolled Nurse as a minimum requirementExperienced in assessing and planning care needs within the communityValid Driver''s LicenceHigh level of computer literacyGood communication skills and personable telephone mannerPossess a ''can-do'' positive attitude and work well in a team environmentExcellent networking, consultation skills to develop new referral sources and relationships with appropriate external organisations.Excellent customer service skills with exceptional verbal and written communication skillsSound knowledge and understanding of home care services and guidelinesIf you would like to work for a fast growing and industry leading company then please apply for this position.
